Supplier Requirements


To be accredited as a vendor/supplier, your company must meet the following criteria:

  • Legally registered and compliant with government requirements
  • Capable of providing consistent and quality products or services
  • Willing to undergo ARKY's evaluation and approval process

How to Become an Accredited Supplier


Follow these simple steps to apply for accreditation:

1) Download and fill out the Supplier Information Sheet

2) Prepare the Following Documents (in PDF):

  • General Requirements
    • Tax Identification Number (TIN)
  • Sole Proprieorship
    • BIR Certificate of Registration (Form 2303)
    • Current Mayor's/Business Permit
    • Bank/Financial Statement
  • Corporation/Partnership
    • BIR Certificate of Registration (Form 2303)
    • Current Mayor's/Business Permit
    • Bank/Financial Statement
    • General Information Sheet


3) Submit the completed form and required documents in the submission form below
